Abstract

Conrad Ramstedt remains one of the most influential figures in Pediatric surgery for his groundbreaking contribution to the management of infantile hypertrophic pyloric stenosis (IHPS). Before his discovery, IHPS was associated with extremely high infant mortality due to the lack of a safe and reliable surgical solution. Ramstedt’s development of the pyloromyotomy, a simple longitudinal splitting of the hypertrophied pyloric muscle without breaching the mucosa, revolutionised treatment by providing a remarkably effective and life-saving procedure. This review article highlights Ramstedt’s early life, clinical background, and the events that led to his accidental yet transformative discovery in 1911. His pioneering technique dramatically reduced mortality, established a new gold standard of care, and set the foundation for modern minimally invasive approaches. The review further explores the challenges he faced, the initial scepticism from the surgical community and the eventual global adoption of his technique due to its safety, reproducibility, and excellent outcomes. Beyond the procedure itself, Ramstedt’s meticulous documentation, clinical insight, and compassion for infants contributed significantly to Pediatric surgical progress. His legacy continues to inspire clinicians, emphasising the importance of observation, perseverance, and dedication to improving child health. Key Message: Conrad Ramstedt revolutionised Pediatric surgery through his discovery of the pyloromyotomy procedure, which transformed the treatment of infantile hypertrophic pyloric stenosis (ItPS). His simple yet effective technique dramatically reduced infant mortality and remains the gold standard even today.
