Dr. Harald Hirschsprung, a pioneering Danish pediatrician of the late 19th century, made a landmark contribution to child health through his detailed description of congenital megacolon, now universally known as Hirschsprung disease. In 1886, he identified a distinct clinical syndrome characterized by chronic constipation, abdominal distension, and failure to thrive in infants, linking these features to a functional obstruction of the distal bowel. His meticulous clinical observations laid the foundation for later pathological discoveries, including aganglionosis of the enteric nervous system, which transformed understanding of gastrointestinal motility disorders in children. Over time, the condition’s pathophysiology, genetic basis, and surgical management evolved significantly, yet Hirschsprung’s original insight continues to guide modern pediatric surgery and neonatology. His work remains a cornerstone in pediatric gastroenterology, representing one of the earliest and most influential examples of syndrome recognition in clinical pediatrics. His pioneering work not only improved early recognition of complex neonatal bowel disorders but also stimulated decades of research that advanced diagnostic techniques, refined surgical approaches, and enhanced long-term outcomes for affected children worldwide. Key Message: Dr. Harald Hirschsprung was the first to clinically describe congenital megacolon, a condition later named Hirschsprung disease. His work established an early model of syndrome identification in pediatrics, based on careful clinical observation
