The aim of obstetrics care is to improve and optimise the outcome of pregnancy. Our objective is to avoid preventable death, disease and disability in children. Cordocentesis, also known as percutaneous umbilical blood sampling, is a critical diagnostic procedure that has significantly advanced fetal medicine. This technique allows for the direct collection of fetal blood, offering valuable insights into fetal physiology, metabolism, and disease. By providing access to real-time data on fetal blood gases, hemoglobin levels, and genetic information, cordocentesis has enabled healthcare professionals to diagnose and manage conditions such as fetal anemia, infections, and genetic disorders with greater accuracy. The procedure has proven particularly beneficial in cases of nonimmune hydrops, severe growth restriction, and structural anomalies, allowing for precise diagnosis and tailored interventions. Furthermore, it has facilitated the correlation of fetal blood data with Doppler ultrasound and fetal heart rate patterns, enhancing the understanding of fetal wellbeing. While cordocentesis carries some risks, including the potential for fetal loss and complications like bradycardia, its ability to provide critical information in highrisk pregnancies has made it an invaluable tool in modern obstetrics. Overall, cardiocentesis has revolutionized prenatal care by offering access to direct fetal circulation thus opening new areas of prenatal diagnosis and therapy and a means of diagnosing and managing fetal conditions that would otherwise be difficult or impossible to detect.

Luteal Phase Defects (LPD), also known as Corpus Luteum Insufficiency or Luteal Phase Insufficiency, are characterized by insufficient function of the corpus luteum, resulting in inadequate progesterone secretion. This can lead to a shortened luteal phase and poor endometrial secretion, which are critical for embryo implantation and pregnancy maintenance. During the menstrual cycle, women with LPD still experience normal follicular development and ovulation. However, due to early degeneration of the corpus luteum or insufficient progesterone, the luteal phase becomes dysfunctional. This can hinder the thickening of the endometrium, which is necessary for a fertilized egg to implant. Luteal Phase Defects are a common cause of female endocrine dysfunctions, impacting fertility and pregnancy outcomes. With advancements in Assisted Reproductive Technology (ART), LPD has become more prevalent, particularly among ART patients. The condition is often addressed through progesterone supplementation or other fertility treatments to improve the luteal phase function and enhance chances of conception. Management of LPD typically includes hormone therapy to correct progesterone deficiency and optimize the uterine environment for implantation.

Asherman syndrome is the presence of adhesions in the endometrial cavity that can result in infertility or recurrent spontaneous abortion with or without amenorrhea. It can present with pain, menstrual abnormalities, recurrent pregnancy loss and infertility. These synechiae may be detected on a hysterogram or sonohysterogram, but are best shown with diagnostic hysteroscopy. Management includes hysteroscopic guided lysis of adhesions and attempt to restore uterine cavity anatomy. Platelet rich plasma, stem cells, growth factors, tissue engineering are being explored for endometrial regenerative therapy. Preventive measures need to be explored more. Efforts are needed in reducing adhesions post instrumentation especially in gravid uterus.

Shoulder dystocia is an obstetric emergency in which gentle downward traction of the fetal head does not lead to delivery of shoulder and additional obstetric maneuver are required to deliver the fetal shoulders. This can cause neonatal brachial plexus injuries, hypoxia and maternal trauma, including damage to the bladder, anal spinchter, rectum and postpartum haemorrhage. Although fetal macrosomia, prior shoulder dystocia,3 excessive weight gain during pregnancy, diabetes mellitus, advanced maternal age, prolonged 2nd stage of labor and oxytocine labor induction increase the risk of shoulder dystocia, most cases occur without warning.5 when shoulder dystocia occurs it should be vocally announcing that dystocia is happening, summoning extra assistance, keeping track of the time from delivery of the head to full delivery of the neonate, and communicating with the patient and health care team are helpful. calm and thoughtful use of maneuvers such as suprapubic pressure, knee to chest (McRoberts maneuver), internal rotation and delivery of posterior shoulder will almost always result in successful delivery. Labor and Delivery teams should always be prepared to recognize and treat this emergency.3

Background: Broad ligament hematomas are rare, but potentially life-threatening complications following vaginal delivery. This can cause concealed hemorrhage and hemodynamic instability, necessitating prompt diagnosis and intervention. Effective management often requires a multidisciplinary approach to prevent maternal morbidities and mortalities. Case: An 18-year-old woman developed a large broad ligament hematoma and hypovolemic shock following a normal vaginal delivery. She was transferred to our tertiary care center on postpartum day one with persistent hypotension, requiring intensive resuscitation and uterine artery embolization. Her condition was further complicated by acute renal failure, disseminated intravascular coagulation (DIC), and respiratory distress, necessitating intubation, multiple blood product transfusions, and hemodialysis. The patient recovered completely with aggressive multidisciplinary management involving interventional radiology, critical care, and nephrology.
