Synchronous malignancies often coincide with esophageal cancer, primarily within the
upper aerodigestive tract. The coexistence of esophageal cancer and renal cell carcinoma (RCC)
is exceptionally rare, with only 12 documented cases worldwide. Esophageal carcinomas
accompanied by synchronous malignancies exhibit a poorer prognosis than isolated esophageal
carcinoma, underscoring the significance of detecting synchronous RCC, as it significantly
influences the choice of treatment strategy.
Presented here is a case involving synchronous esophageal and RCC in a 62-year-old Indian
male. Following a review of literature, we propose that this case is likely the first reported
instance from India. The patient has been scheduled for radical nephrectomy, followed by
preoperative chemoradiation and esophagectomy.
This case report holds potential relevance in the expanding realm of research on possible
carcinogens in synchronous malignancies development. The simultaneous onset of esophageal
malignancy and RCC may be linked to the ingestion of a carcinogen excreted by the kidneys.

Carcinoma of the prostate metastasizing to the penis is rare. These patients have a poor prognosis receiving various treatment modalities. This report describes a rare case of penile metastasis from prostate cancer, indicating that clinicians need to improve their understanding and discrimination of this disease.
 

Background: Biphenotypic Sinonasal Sarcoma (BSNS) is a recently described tumor with
its site of origin limited to the nose and paranasal sinus. It is a rare low-grade sarcoma
characterized by concomitant neural and myogenic differentiation with high recurrence rates
but lacks metastatic potential. Limited literature is available about the standard treatment
regimens due to their rarity.
Case Presentation: Herein we describe the details of the case of a 55-year-old male,
known case of recurrent biphenotypic sinonasal Sarcoma who presented with bilateral nasal
obstruction. The diagnosis was confirmed by immunohistochemistry. He underwent surgery
but developed recurrence twice, and later treated with Radiation Therapy.
Conclusions: Mainstay of the treatment remains surgery but due to its rarity, no definite
treatment guidelines are defined. Radica or adjuvant treatment with chemotherapy and/or
radiotherapy have been tried with limited data on the outcome.
 

Normal cells and cancer cells can be differentiated clearly during insulin induced
hypoglycaemia because of the difference in number of insulin receptors on cell membrane as
well as the biologic response which insulin modifies. This helps in targeting the chemotherapy
drugs more specifically and effectively inside the cells. Under insulin this occurs in cancer
cells with low doses of chemotherapy drugs with lesser side effects. With this aim we did
this randomised study in advanced head and neck cancer cases comparing low doses of
cisplatin under hypoglycaemia with usual weekly dose of cisplatin along with full dose of
radiotherapy. There was 77.7% complete response and 11.1% partial response in low dose and
90% complete respone in full dose chemotherapy with minimal side effects in low dose arm
and no neurologic or cardiac toxicity was observed in hypoglycaemia arm. Thus we conclude
that the use of low dose chemotherapy is equally effective with minimum side effects when
given under hypoglycaemia.

ntroduction: Central Nervous Tumors constitute 3% of cancer cases worldwide. The
incidence of CNS tumors in India ranges from 5-10 per 100,000 population, ranking 14th among
all other tumors and accounting for 2% of malignancies. The five most frequent tumors were
astrocytoma (47.3%), Medulloblastoma (11.4%), craniopharyngioma (9.7%), ependymal tumors
(4.8%), and nerve sheath tumors (4.1%). The tumor spectrum varies among different ages and
sexes. Materials and Methods: All the patients of CNS tumours who attended the Department of
Radiation Oncology, SAMC, and PGI from October 2010 to December 2019 are included in the
study. The medical records of 238 patients were evaluated, out of which only 102 were fit for
the study as per the inclusion and exclusion criteria. 

Results: The 3-year median overall survival (OS) of 8 years and Progression Free Survival
(PFS) of 7 years, while the 5-year median OS of 9 years and PFS of 7 years is recorded.
Glioblastoma (GBM) is the most aggressive out of all histologies, with a 2-year survival of
31% and a median OS of 8 months, as per our study. We got some good results with brainstem
glioma as well, with a 2-year survival of 40% and a median OS of 18 months.
Conclusion: The survival in CNS tumours has improved widely with adjuvant concurrent
chemoradiation postoperatively, and the better the resection is done, the better the survival is
seen.
