Background: Kalanchoe pinnata, an important medicinal plant of the stonecrop family, is widely distributed in India and tropical and subtropical parts of the world. It has been reported for its folkloric use in various disorders such as abdominal discomfort, boils, bruises, cholera, cuts, diabetes, diarrhea dysentery, flatulence, headaches, kidney stones, indigestion, insect bites, scabies, sores, urinary insufficiency, wounds. Results: The present review has focused on the botanical description, phytochemistry, and ethnomedicinal and traditional uses of Kalanchoe pinnata along with its reported pharmacological activities. Chief chemical constituents and pharmacological aspects of Kalanchoe pinnata have been deeply explored to unravel the unexplored folklore/ ethnomedicinal uses of this plant so that the researchers working on this plant may be able to find new insights to continue the further investigation on this plant. The pharmacological aspects like anti-diabetic, anti-inflammatory, anti-nociceptive, anti-diarrheal, anti-pyretic, and anti-cancer potentials evaluated by various in vitro/in vivo methods on this plant have been reported. Conclusion: Various traditional uses have been reported that need to be scientifically investigated in-depth and several pharmacological activities have been reported for the Kalanchoe pinnata, but more detailed and mechanismbased studies linked to a particular lead compound need to be targeted in the future. Moreover, this plant has not been completely assessed based on its safety and efficacy in humans. It is expected that this review will compile and improve the existing knowledge on the potential utilization of Kalanchoe pinnata in complementary and alternative medicine.

Non healing wounds are a major problem all over the world. Many therapies have been introduced for the management of chronic non healing ulcers. It is always challenging to manage them. There is no well established method that accelerates the wound healing rate. Daridorexant, a double orexin type 1 and type 2 (OX1 and OX2) receptor antagonist (DORA), has been recently introduced in the market for the treatment of insomnia. DORAs are a new form of sleep drugs that can be used instead of standard positive allosteric gamma-amino butyric acid (GABA)-A receptor modulators. Daridorexant blocks the effects of the wake promoting orexin (also known as hypocretin) neuropeptides by binding specifically to both orexin receptors. This approach avoids the extensive inhibition of neuronal circuits that positive allosteric GABA-A receptor modulators cause, as well as the related adverse effects. In this reviewdetailed Pharmacology of Daridorexant including its mechanism of action, Pharmacodynamics, and Pharmacokinetics have been compiled.

Combination of intra thecal local analgesic and opioid are commonly used for neuraxial blocks. This combination gives excellent intraoperative as well as postoperative analgesia. Pruritus is a troublesome side effect of intrathecal opioids. Sometimes it may be more unpleasant than pain itself. We experienced a case of severe pruritis evoked by intrathecal fentanyl in a patient with Intertrochanteric fracture (Rt) femur for ORIF. After 90 minutes of intrathecal administration of a combination of bupivacaine and fentanyl she began to complain of severe itching which was managed with Inj Hydrocortisone Succinate. After surgery patient was shifted to ward for close monitoring. General isedprurit is and tachypnoea recurred after 18 hours which responded only to naloxone infusion for 6 hrs. Therefore it is suggested that naloxone is useful to reverse the effect of fentanyl inducedpruritis.
