Systemic diseases such as diabetes mellitus, rheumatoid arthritis (RA), and chronic kidney disease (CKD) often have musculoskeletal complications that significantly impact patients’ quality of life. These complications necessitate a multidisciplinary approach involving orthopedic surgeons and other specialists to ensure early recognition, intervention, and optimized patient care. An interdisciplinary approach enables the integration of systemic and musculoskeletal health strategies, leading to better patient outcomes. For instance, diabetes-related Charcot arthropathy and osteomyelitis require both orthopedic and endocrinology care, while RA demands immunosuppressive therapies alongside orthopedic interventions. CKD, due to disruptions in calcium and phosphate metabolism, increases fracture risks and requires nephrology and orthopedic collaboration. Emerging technologies such as telemedicine and precision medicine further support collaborative care. This article underscores the importance of interdisciplinary teamwork in addressing the musculoskeletal manifestations of systemic diseases.
 

Nanotechnology and stem cell therapy (SCT) have emerged as revolutionary approaches in musculoskeletal disorders, offering innovative solutions to complex disoders. Traditional orthopedic surgeries and pharmaceuticals often fall short in terms of long-term efficacy and recovery time. Nanotechnology and stem cell therapy present more advanced, targeted, and minimally invasive alternative approach.  Nanotechnology involves the manipulation of materials at nanoscale to create systems that can deliver correct diagnosis, and treatment of musculoskeletal disorders. Nanomaterials, such as nanoparticles, nanotubes, and nanofibers, have depicted significant improvement and enhancement in bone regeration, drug delivery, and tissue repair. small size of nanoparticles allow them to integrate better with biological system, ensuring precision in delivering therapeutic agents to specific sites in order to minimizing their side effects. Interestigly, nanoscale scaffolds have been developed to promote bone and cartilage regeneration by mimicking the extracellular matrix (ECM) and supporting the growth of new tissues.  On the other hand, SCT uses undifferentiated cells with the ability to transform in to specific types, offering significant role in the treatment of damaged tendons, cartilage and bones. These stem cells can be guided more effectively to the target areas with the  combination of nanoparticles, increasing the success rate of treatments.
 

Introduction: Septic arthritis is a dreadful condition affecting joints which warrants an emergency arthrotomy in tandem with debridement followed by intravenous antibiotics during the peri-operative period. It generally affects larger joints, namely the hip, knee, shoulder, ankle and elbow. Serratia marcescens causing septic arthritis is uncommon in an immunocompetent individual and the available literature on the same, is limited.  Case presentation: A 73-year-old female diabetic patient, had presented to the casualty with swelling in the right knee with local rise of temperature and limited movements for five days. She had undergone left total knee arthroplasty four months ago with no postoperative complications and was ambulating well. The radiographs did not show signs of gross abnormality. Synovial fluid aspirated from the affected knee was found to have Serratia marcescens infection localized to the joint, as the blood culture was negative for the same organism. Left knee arthrotomy with debridement was done along with polyethylene liner exchange and local antibiotic delivery via calcium sulphate crystals mixed with vancomycin, tobramycin and gentamicin. Following this, she was given intravenous antibiotics based on the culture and sensitivity   report.  Conclusion: Septic arthritis of the knee joint caused by Serratia marcescens is a rare occurrence, especially following total knee arthroplasty. Hence special attention had to be given for surgical planning and treatment regimen. Knee effusion with painful limitaion of movements is a pathognomonic sign of a septic knee. Initial antibiotic cover remains the mainstay of the treatment but in an acute setting, knee arthrotomy with debridement and application of local antibiotic delivery gave excellent results.  

The terrible triad of elbow, initially named by Hotchkiss, is a combination of dislocation of elbow, along with the fracture of radial head and coronoid process. The management of such injuries is to restore the stabilizing bony structures of the elbow, if not treated appropriately can lead to stiffness, post-traumatic arthritis and pain. So, the sequence of fixation is mobilization of the radial head fragments and fixation of coronoid fracture, fixation or replacement of radial head or neck fracture, and reattachment of the Lateral Collateral Ligament origin to lateral epicondyle. We are reporting one such case, of a male of 50 years, and have discussed mechanism of injury, clinical presentation, management and prognosis.
 

Orthopaedic education in India has evolved from traditional apprentice-based learning to a structured and standardized approach. Despite advancements in curricula, inconsistencies persist due to unequal access to resources and variations in training quality across institutions. Premier institutions often provide access to advanced technology, while smaller or rural facilities may lack the infrastructure necessary for comprehensive training. This disparity affects residents’ exposure to specialized procedures and research opportunities. Additionally, the emphasis on research within orthopaedic education remains limited, with challenges including insufficient mentorship and time constraints. To address these issues, it is essential to integrate simulation-based training, particularly in resource-limited settings, to enhance surgical skills development. Furthermore, the implementation of formative assessment methods, such as OSCEs and workplace-based assessments, can provide continuous feedback, improving both practical skills and professional competencies. The integration of emerging technologies like telemedicine, 3D printing, and virtual reality can bridge the gap between theory and practice, enriching the learning experience. Reforms in these areas are necessary to ensure that all residents receive high-quality education, equipping them with the skills needed to excel in a dynamic healthcare landscape. A collective effort from government bodies, institutions, and faculty is required to overcome current challenges, enabling Indian orthopaedic education to meet global standards.
