Trigeminal neuropathy a chronic or recurrent pain in the area of trigeminal nerve distribution caused by trauma or injury. The incidence following injuries to the peripheral branches of the trigeminal nerve following implants, 3rd molar extractions, orthognathic surgery, mid-face fractures and root canal therapy is around 3–5%. Considering the wide prevalence of such injuries and procedures PTTN is suspected to be common. Although it may occur at any age typical age of onset is around 50 years, and patients are largely female. Prognosis of PTTN is poor. There is a limited response to available and recommended pharmacotherapeutic interventions. Peripheral surgical interventions aimed at pain relief are generally contraindicated; however microsurgical nerve repair may improve sensation. But whatever the line of treatment is there is having its own merits and demerits with limitations. Hence undoubtedly there is a need for an integrated therapeutic approach in PTTN. In this regard an effort was made to treat the PTTN and the study showed good results in the management of PTTN with a course of Hanubasti and Nasya along with some internal medications. A case report of 52-year-old female who presented with complaints of burning sensation and dull aching pain over the right side of the temporal, pre auricular, infraorbitaland maxillary regions, pain in temporomandibular joint after chewing, continuous and loud talking, continuously since 2 years has been presented here.

Seborrheic dermatitis is a common dermatological disorder which affects the scalp and other seborrheic areas on the face, upper-chest, axillae, and inguinal folds. Its incidence peaks during three age periods — in the first 3 months of life, during puberty, and in adulthood with an apex at 40 to 60 years of age. It is also called as dandruff. Darunaka is the non-inflammatory type of seborrheic dermatitis. Darunaka is one among the nine kapalagata rogas explained by Acharya Vagbhata. It is characterized by Kandu (itching), Keshachyuti (falling of hair), Swapa (loss of touch sensation), Rookshata (roughness or dryness of the skin) and Twaksphutana (scaly skin of the scalp). In Ayurvedic classics different treatment modalities are told for the management of Darunaka, like shiro abhyanga, shirolepa, shiro basti, etc. Shiro abhyanga is one of the topical treatments, helps to pacify the vitiated local doshas and get rid of the problem. Here is a case report of seborrheic dermatitis where a male patient aged about 42 complains of itching of scalp, hair fall, red patches, flakes over the head and shoulder, since 2 years has been presented here. After the course of treatment there was marked improvement on symptoms like itching of scalp, hair fall and flakes.

Shad kriya kala is an Ayurvedic concept which describes the various stages of disease formation. Based on the stages of diseases preventive measures can be applied to overcome the complications. Acharya Sushruta, Madhava and Charaka have described the concept of Shad kriya kala in Mutrakrcchra, Amplapitta, Kasaroga which explains the disturbance of Doshas. Basically Shad Kriya kala is an objective approach of Ayurveda Acharyas helping for the clinical practice. Early diagnosis of diseases helps to cure the diseases in early stages without any complications.

Osteoarthritis is a degenerative disease affecting weightbearing joints usually seen in the elderly people and the people who do excessive physical deeds. Ayurveda explains many bahyaparimarjana chikitsa (external therapies) for Sandhigata vata like Daha, Sweda, Sneha and Upanaha. Upanaha sweda or poultice application is one of the treatments described in the management of Sandhigata vata and inflammatory swelling. Pain management is one of the emerging areas of research and development in Ayurveda. Upanaha sweda or Poultice is proved effective in the management of pain and other conditions where external treatment is employed. Here an attempt is made to explain about role of Upanaha sweda and its mode of action in niram Janu Sandhigata vata.

Incidence of Diabetes Mellitus (DM) is rapidly rising at an alarming rate all over the world and in Sri Lanka. It downgrades the quality of life of people. Management of Diabetes Mellitus is one of the challenging health issues faced by health professionals. Hence, alternative systems of medicine are being explored to fill the gap of its management. Prameha has been described in Ayurveda for thousands of years ago. Apathya nimittaja prameha (ANP) is caused by unhealthy dietary and lifestyle factors and it is well correlated with type II DM. Ayurveda possesses a number of valuable remedies that can be used in the management of ANP. The objective of the present study is to evaluate the effect of Hemadrumasyadi Decoction (HD) on Type II DM. Selected patients (n=30) were treated with HD 120 ml, with 10 ml of bee honey before meals, twice a day for four weeks. All the patients were assessed for quantitative and qualitative parameters, before and after the treatment to evaluate the effect of decoction. Data were analyzed with SPSS software. Present study has revealed that Type II DM is more common among 40–60 age group (78%) and people who engaged in mental labour dominant occupations (74%). Sedentary life style (72%) and excessive intake of fatty and sweet foods have found to be the leading causative factors. Pitta-Kapha type of prakriti is more prone to have ANP (63.3%). HD produced highly significant (p<0.001) improvement in polyuria, polydipsia, fasting blood sugar (FBS), total cholesterol and low density lipoprotein (LDL) whereas significant (p<0.05) improvement in fatigue, burning sensation in hands and feet, high density lipoprotein (HDL). Finally, it is concluded that Hemadrumasyadi Decoction is effective in improving some parameters of Type II DM significantly.

Sasyaka Bhasma (calx of Sasyaka, i.e. Copper sulphate or Blue vitriol), a unique mineral product and one among the Maharasas of the Rasashastra (Ayurvedic Pharmaceutics or Indian Iatro-chemistry), due to its wide therapeutic utility; it is used in several formulations. Sasyaka Bhasma, a unique mineral product used traditionally in the management of Kushta (Leprosy), Hrudroga (Heart diseases), Switra (Vitiligo), Shula (Abdominal pain), etc., was prepared according to the prescription in the Ayurvedic classics and subjected to various bhasma pariksha (tests for calx), including the Namburi Phased Spot Test (NPST), one of the qualitative tests described for various Ayurvedic preparations. NPST helps to differentiate between, and thus identify, various bhasmas (calx). It depends upon the pattern of the spot, which develops after a specific chemical reaction. Sasyaka Bhasma prepared by classical reference in our department was subjected to abovesaid tests and the results were compared with standard protocol. The prepared bhasma had shown the nearest results to standard NPST of Sasyaka Bhasma.

The present study is a clinical study to evaluate the efficacy of Mahapasmul Decoction (MD) on Sthaulya (Overweight and Obesity). Diagnosed patients (30) were selected from Ayurveda Teaching Hospital at Borella, Sri Lanka from the period of January 2015 to May 2016. Patients who had BMI between 25-40 kg/m2 were included in the study. Subjective assessment criteria were the symptoms of obesity with proper grading whereas objective criteria were BMI, body circumferences, skin-fold thickness, lipid profile and FBS. Data were analyzed by using SPSS statistical software. In this study, Mahapasmul Decoction was given for eight weeks, at the dose of 120 ml twice a day, before meals with 5 ml bee honey. Mahapasmul Decoction reduced most of the signs and symptoms of obesity, most of the body circumferences, BMI (from 35.30 ± 0.85 to 32.53 ± 0.92), skin fold thickness, triglyceride (from 122.73 ± 1.68 to 126.45 ± 1.10) and fasting blood sugar (from 95.68 ± 1.67 to 91.91 ± 1.54) in statistically highly significant manner (p<0.001) whereas the improvement of excessive thirst, excessive sweating and drowsiness is significant (p< 0.05). Collectively, MD is composed of katu (20%), tikta (100%) and kasaya rasa (100%); laghu (100%), ruksha (66.6%) and guru guna (16.6%); ushana virya (66.6%); and katu vipaka (100%). The pharmaco dynamic properties of MD reduce kapha and meda and increase agni. Hence, MD is efficacious on promoting digestion and correcting sroto-avarodha. Finally, it is concluded that Mahapasmul Decoction is effective on most of the subjective and objective parameters of Sthaulya (Overweight and Obesity).

The present study was aimed at evaluating comparative effect of Erandadi and Rasonadi Decoctions on Amavata. 40 patients of Amavata were randomly selected from the OPD of Meegoda Ayurveda hospital and equally divided into two groups: group A and group B. The group A was treated with Erandadi Decoction (ED), and group B was treated with Rasonadi Decoction (RD) for a period of one month and followed up for a one month. Data were analyzed by SSPS statistical software. The majority of patients of Amavata were female (95%) and in 46–50 age group (52.5%). Clinical features of Amavata were aggravated by cold water (75%), walking (60%) and rainy weather (57.5%) while it was relieved by rest (75%). Considering prakriti, majority of patients had vata-kapha prakriti (60%). ED improved joint pain, tenderness and disturbed sleep in highly significant manner (p<0.001) whereas stiffness, loss of appetite, heaviness and thirst was improved in significant manner (p<0.05). ED improved ESR, pulse rate, WBC and lymphocyte count in highly significant manner (p<0.001). RD improved joint pain, stiffness, tenderness and loss of appetite in highly significant manner (p<0.001) whereas disturbed sleep, heaviness and thirst was improved significantly (p<0.05). RD improved pulse rate, WBC and lymphocyte count in highly significant manner (p<0.001). When comparing the effects of ED and RD, it was evident that RD is more effective over ED in improving subjective as well as objective parameters of Amavata.
