Objectives: To treat Active Tubotympanic CSOM (Pootikarna) case with Ayurveda Therapeutic Procedures and Shamanoushadhis.

Method: The present case of Active Tubotympanic CSOM approached to OPD of KAHER BMK Ayurveda hospital with a complaints of Otorrhoea, Otalgia, Deafness, in Right ear Since 2 years and Headache, Running nose since one month. Symptoms were transient and severity increased since last 1month. Oto-scopic findings were documented in the form of the photographs. Ayurvedic Treatment Protocol is planned as per the disease condition which was included local therapy in the farm of Karna and Nasa Dhoopana with Haridra and Ghrita varti along with Steam inhalation for 7 days along with Haridrakhanda (2tsf) and Triphala Guggulu (2 tabs) orally twice daily after food.

Results: There is significant decrease in the symptoms of CSOM viz. Otorrhoea, Otalgia, Deafness etc. and also major clinical changes pertaining to the size of perforation of Tympanic membrane seen in Otoendoscopy images.

Conclusion: Chronic Suppurative Otitis Media (CSOM) causes recurrent or persistent discharge (Otorrhoea) through a perforation in the tympanic membrane and can lead to thickening of the middle ear mucosa and may lead into complications such as mucosal polyps & cholesteatom. CSOM can be correlated with the Pootikarna which can be cured through Ayurvedic therapeutic procedures and oral medications, which is cost effective and without any adverse drug reactions.

Background: Only a standardised good quality educational hospital can be good tool for quality education. Despite of Minimum Standard Requirement Regulations prescribed by the Central Council of Indian Medicine (CCIM) to Ayurvedic hospital and regular inspection by regulatory body Ayurvedic education system still needs improvement in its quality education. Private hospital establishment acts of state government also yet to achieve in quality medical care. NABH Quality accreditation to college hospitals gives better learning tools for medical students.

Aims & Objective: Implementation of principles of NABH in Ayurvedic college hospitals increases quality of Ayurvedic education.

Materials & Method: Chapter by Chapter guidelines of NABH, like Infrastructure, Documentation, human resource Management, Standard operation procedures, facility management systems, etc protocols are analysed for improvement in learning methodology in education.

Discussion: As NABH is autonomous assessment body which prescribes and frames quality guidelines to health care system, is most trusted organisation in issuing quality accreditation. Lacuna in regulatory body in assessment of quality of health care system in Ayurvedic college hospital may be assessed by NABH which in turn helps the apex body, to grant permission to its affiliated colleges and to improve the quality of education.

Conclusion: Principels of NABH helps to adopt standard operating procedures (SOP), Good clinical practices (GCP), Good Laboratory Practices (GLP) etc. in turn acts like ideal tools in learning methodology.

Ano Rectal Polyp (ARP) is the local tissue malformations and polyp is a mass of tissue arising from the mucosa and protruding into the Gastro-intestinal lumen. It is a bright red/whitish glistening sphere usually of the size of a pea or more. The delay in approach of treatment becomes sometimes grave concern i.e. malignant transformation. Most of polyps are asymptomatic except for minor bleeding which is usually occult having no potential to be malignant. It is common in children’s age. Though the exact cause for polyp is not known but physiologically the growth and division of new cells is usually regulated by wear and tear process of the body; but in some cases the new cells grow and divide before they are damaged and required. Such excess growth causes to form polyp. The risk factors commonly go to the persons of age over 50, over weight, uncontrolled type-II diabetes, history of- polyp suffering in past ovarian or uterine cancer and life style behaviors like smoking, drinking alcohol regularly, sedentary life style, consuming high fat diet regularly may contribute to growth of polyp. Fresh bleeding, mucous discharge per rectum is cardinal symptoms while prolapsed mass/polyp during defecation is alarming feature. Bleeding may cause anemic. In complication a rectal polyp very rarely initiates Intussusceptions. The surgical treatment is only option where the reoccurrence is possible, hence not successful. Finding the alternate, Para surgical- curable method, the PCA therapy becomes successful with follow up of 2 years having the treatment of 41 cases of polyps alone and 67 cases of polyps with multiple anal diseases. No adverse effect was noticed. The PCA therapy is safe, simple, low cost effective and noninvasive method.

PrathamapatalagataTimira is one among the stages of Timira described under Drishtigata Rogashaving the chief symptom of avyaktadarshana. It can be correlated with Simple Myopia or short sightedness which is a variety of refractive error producing blurred vision for distance and may or may not be associated with asthenopic symptoms. If appropriate treatment is not done in this stage it progresses further and may lead to blindness. In modern ophthalmology it is managed with the use of refractive aids and surgeries for improving the quality of vision with limited scope in curtailing the rogression of the disease. In this regard this study is been taken up to evaluate the effect of Tarpana with Bhaskaradi Ghritaonce a day for 5 days followed by a gap of 7 days after the procedure. Later tarpana was resumed for another 5 days. Similarly this procedure was repeated for 3rd and 4th sittings with total period of 48days. Pre and post treatment assessment of the patient was carried out with the subjective parameters like avyaktadarshana, headache, eyestrain, watering of eyes and objective parameter like Visual Acuity with Snellen’s Chart. These were suitably graded to assess the results that were statistically analyzed and it showed significant results.

Haratala Bhasma (calx of Haratala i.e. Arsenic Trisulphide - As2S3), is categorized under Uprasa category of the Rasashastra having some peculiar attributes. Haratala Bhasma has its therapeutic indication in many of the diseases mainly Shwas (respiratory disease), Kasa (cough), Apasmar (epilepsy), Kushta (leprosy), Vatarakta (gout), Firang (syphilis) etc. which are now established free radical mediated diseases. It is also having properties like Vrushya (aphrodisiac), Rasayana (immunomodulator). Review of the current literature available on Rasayanas indicates that Anti-oxidant and Immunomodulation are the most studied activities of the Rasayan drugs. The effect Immunomodulation has on the human body can be compared to some extent with the effect of Rasayan dravyas, given in various Classical texts. Antioxidants and Immunomodulators are talk of the hour since a decade or two. Lot of research has been carried out in this regard but the need of them is not yet fulfilled. Hence assuming Haratala Bhasma possesses the anti-oxidant and Immune-modulators properties the present study was carried to validate it scientifically: Haratal was subjected to shodhan (purification), maran (incineration) as per Rasatarangini. Bhasma was subjected to physico chemical analysis and advanced analytical techniques like AAS, NPST, XRD and SEM etc. Screening of free radical scavenging activity was done using albino rat liver homogenate (ex-vivo) and with standard parameters like LPO, SOD, GSH and CAT. Immuno-modulatory effect was screened using three different healthy adult human blood samples with four parameters like Nitro-blue tetrazolium (NBT) assay for respiratory burst. Phagocytosis and candidacidal assay for phagocytic function and Chemotaxis to assay motility of neutrophills. Three different concentration of the test drugs (1%, 2%and 5%) were used for both screening. The results were self conclusive and indicated that 2% and 5% Haratala bhasma suspensions showed better results in free radical scavenging activity in 2 days and 4 days study in all the four parameters LPO, SOD, GSH and CAT. The immuno-modulatory screening showed Haratal bhasma has a better effect at a concentration of 5% when compared to 1% and 2% concentration. Thus, the study revalidates the reference of Haratala Bhasma as a Rasayana and hence also establishing it as an Immunomodulator.
