Numerous revision procedures such as surgical excision, intralesional steroid injection, cryotherapy, dermabrasion, soft tissue augmentation, chemical peeling and laser therapy are available for the correction of various types of scars. Erbium:YAG lasers are successfully used to treat a variety of epidermal and dermal lesions, including rhytides, dyschromias, and certain types of scar. Recently we came across the usage of Erbium :YAG laser for the management of scar following the harvest of STSG.

The increasing physical, mental, societal, and financial challenges posed by wounds and their care for individuals, families, and society underscore the urgent need for attention from the healthcare sector. Various factors contribute to the shifting landscape of healthcare provision for patients dealing with chronic wounds, including managed care dynamics, the scarcity of wound care specialists, a growing elderly and disabled population, regulatory concerns, and legal liabilities. Physicians also grapple with complex issues in managing chronic wound cases due to their time-intensive nature, high-risk profile, lack of profitability, and legal implications. Telemedicine emerges as a valuable tool to facilitate communication between physicians and specialized wound care experts. Utilizing digital imaging for skin lesions offers a safe, precise, and cost-efficient means of referral. Telemedicine employs two primary modes—store and forward (asynchronous transfer) and real-time transmission (synchronous transfer, such as video conferencing)—in the wound care context. In the hands of experienced clinicians, telemedicine technology can streamline the management of challenging wounds. While apprehension about technological change persists, the evolution of telemedicine in wound care has demonstrated a foreseeable process of advancement. In this case report, we have reported a case of 8 year old male child who sustained electrical burns over scalp, and was discharged later following wound healing. The child later developed pustule over the scalp which was picked up immediately and attended to, because of serial follow up everyday following discharge, because of telemedicine

Tangential excision followed by immediate coverage using split skin grafts is considered the standard approach for treating deep burns, but it comes with its drawbacks. Challenges arise when there’s a lack of donor area, and the grafts often lead to contraction and suboptimal aesthetic and functional results. While various biological and cellular substitutes have been developed, they tend to be costly and not always suitable for routine use in our demographic. Here, we present a new technique utilizing a collagen and dermal extract scaffold from full thickness skin graft (Fig. 3) obtained from abdominoplasty (Fig. 2) of the same patient for wound coverage. This scaffold offers safety, ease of preparation, affordability, and effectiveness in managing raw areas post-procedure.

Despite many scientific advancements, we haven’t found satisfactory treatment for many dermatomes. The disFig.ment caused by these conditions is a source of distress. Pathophysiology of certain dermatomes roots from common pathophysiology. Though lots of medical treatments are available, however, they provide transient relief. Symptoms often re-appear after sometime.The conditions have a negative effect on overall quality of life of the patients leading to anxiety and depression in some cases. Usually these conditions take a long time to be asymptomatic only to relapse later. Janus kinase inhibitors (Jaknibs) are novel medications approved to be used in rheumatological conditions. Increasing evidences have shown their efficacy in many dermatological conditions such as psoriasis, vitiligo, atopic dermatitis, alopecia areata and a few others. Tofacitinib was first approved in 2012 by USFDA for the management of rheumatoid arthritis. Tofacitinib inhibits JAK1 and JAK3, reducing the signaling of pro-inflammatory cytokines. This inhibition affects various immune processes, which is beneficial in treating inflammatory and autoimmune conditions, including those affecting the skin.The drug is being accepted in dermatological conditions. In many other dermatoses, JAK inhibitors carry a promise in management. Cutaneous graftversus-host disease, erythema multiforme, dermatomyositis, chronic actinic dermatitis, and lupus and hypereosinophilic syndrome are few of such conditions. In the present review, we discuss the literature and evidence of the use of tofacitinib in dermatological conditions.

Aloe-vera is well known for the cost effect positive actions it has on the skin, with a very minimal risk profile. The anti-inflammatory, wound healing & anti-ageing effects of aloe-vera on the skin have been extensively studied & discussed. Similar effects may be demonstrated & observed on burn wounds. We tested the aloe-vera gel on a patient with electric burns post split skin grafting at the donor site& observed the progression of the wound. Unsurprisingly, we observed that wound healing was considerably enhanced by the aloe-vera gel application

Skin grafting frequently results in scarring at the donor site. A number of factors, including the patient’s age and gender, have previously been connected to donor site scar satisfaction. Patient opinions regarding scarring at donor sites are not well documented. Physicians may be able to provide perioperative care that reduces the possible negative effects of donor site scarring if they have a thorough understanding of patient attitudes and the associated factors. Our study aims to identify factors associated with donor site scarring satisfaction and investigate patient perceptions of their scars using quantitative scar assessment tools.
