Topical application of autologous platelet rich plasma (APRP) is used in the treatment of various types of wounds. Currently available methods for application of APRP include injectable or irrigation by syringe but have the limitation of inadequate coverage of surface area requiring large amount of APRP. To improve large surface area with minimum amount of APRP we have designed indigenously made spray device. This article highlights the preliminary pilot application of this device in one of the patient in plastic surgery. This device is easy to prepare and cost effective.

Chicken pox scar is one of the common causes for facial scarring. Facial scars have been found to have a significant impact on their psychological well-being and have been associated with depression. Multiple treatments are available for chicken pox scars, which include dermabrasion, microdermabrasion, laser treatments, dermal fillers, chemical peels, micro needling and the combination therapy. We report our experience with chicken pox scar treated by volume enhancement with autologous fat injection followed by microdermabrasion.Depressed part of the scar was taken care by adding Volume (lifting) with fat grafting and pigmentation component of the scar was taken care by microdermabrasion.

Buschke Lowenstein tumour (BLT), also known as giant condylomata acuminate is a rare locally invasive tumour. This cauliflower-like lesion of the anogenital region often arises from a pre-existing warty lesion. It is locally destructive despite a relatively benign appearance on histopathology. It carries a high recurrence rate and a significant potential for malignant transformation into verrucous squamous cell carcinoma. While wide surgical excision has been the mainstay of therapy, use of combination treatments is gaining ground. However, patient-specific issues and complications often render ideal treatment infeasible, with need for innovative approach to therapy. We report the case of a young Indian man with BLT with dysplastic changes, treated successfully with 16-week long monotherapy with 5% imiquimod cream, owing to unsuitability of traditional therapies in his case.

Neurofibroma is a benign condition with tendency to recur. Post excision aesthetic appearance and primary closure without tension are concerns. This article highlights the technique by which neurofibroma can be excised and closed primarily without tension and aesthetic acceptable appearance

Chemical peeling remains the mainstay of dermatological procedures for rejuvenation and superficial resurfacing with indications ranging from acne, rosacea, melasma and various other facial melanoses, acne and post-traumatic scars and photoaging. Scientific research to develop new peel molecules or combinations to enhance efficacy and minimize adverse effects is ongoing. In this short review, we discuss some recent advances in the therapeutic armamentarium of chemical peels appropriate for the skin of color. Lipohydroxy acids (LHA), polyhydroxy acids (PHA), combination peels consisting of hydroquinone, acetic acid, glutathione as the most active component have been developed and delivering good results in various indications. Home-based peels based on PHAs like lactobionic acid or peel-impregnated pads are another novelty addition to the chemical peel kit of a dermatologist. It is important for all dermatologists and physicians practising chemical peels to be aware of these new peel formulations and their protocols to deliver efficacious and safe outcome to their patients.

 Laser toning using low-fluence 1064-nm QSNY is being increasingly used to target photoaging and hyperpigmentation in darker skin types. However, sub-optimal response stemming from the use of low-fluences, as well as development of complications especially punctate leukoderma are also being increasingly reported with this approach. Since the development of the fractional- lasers, the fractional mode has been successfully incorporated with QS Nd: YAG (QSNY) laser (1064 nm) systems. Fractional mode lasers have the advantage of sparing certain areas in between the treated areas called microscopic treatment zones (MTZs), allowing for rapid healing, shorter recovery times, and less post-inflammatory complications. In this case series, we report our experience of a novel US-FDA approved fractional QSNY laser device Helios III in the treatment of various cutaneous lesions disorders in Indian patients including melanocytic nevi, atrophic scars, pigmented scars, laser hair reduction, reduction of white hair, melasma, and toe nail onychomycosis.

Background: Alopecia areata (AA) is a recurrent, non-scarring type of hair loss. Although its etiopathogenesis is not fully understood, imbalance of some trace elements in the blood has been suggested as a possible contributory cause. Objective: To evaluate the role of serum levels of zinc, copper, magnesium and iron in the etiopathogenesis of AA. Methodology: This case control study was carried out in 40 patients with AA and 40 healthy volunteers who served as controls. Clinical assessment was performed for every patient. A 5 ml venous blood sample was collected from every patient and control, centrifuged and sent for quantitative detection of serum levels of zinc, copper, magnesium and iron by atomic absorption spectrometer. Results: The mean value of serum zinc was significantly lower, and the mean values of serum copper and magnesiumwere significantly higherin AA patients than the control group (p<0.001). The serum copper/zinc ratio was significantly higher in patientscompared with the control group (p<0.001). The mean value of serum iron was higher in AA patients compared to the controls; but this finding was not statistically significant (p=0.632). Further, the serum level of zinc was found to be significantly affected by patients’ sex, age and duration of disease. Conclusion: Elevated serum copper and depressed serum zinc levels could be one of the contributing factors of AA. Decreased serum zinc level may additionally have a role in prolongation of the disease course. The role of magnesium levels need further elucidation. Serum iron levels do not seem to contribute to the pathogenesis of AA.
