Oral submucous fibrosis is chronic, insidious disease affecting the oral cavity and sometimes the pharynx and rarely the tongue. A 20 year old patient with oral submucous fibrosis was treated with contact diode laser under local anesthesia and was followed up for 6 months. Diode lasers offered excellent results and had many advantages over conventional surgical treatment.

A mesiodens frequently delays and interferes in the eruption of maxillary central incisors. In the present case, an unerupted mesiodens caused rotation of the permanent left maxillary central incisor. Early detection of mesiodens can prevent occlusal, a esthetic, and psychological problems in young children. Moreover, an early treatment provides for better retention. An early surgical intervention was done to extract the mesiodens and the affected tooth derotated using a semi-fixed appliance. Extraction of mesiodens in early mixed dentition period followed by timely correction of the affected permanent tooth is recommended, thereby lessening the chances of a more complex surgical and orthodontic treatment in future.

A talon cusp is an uncommon anomaly in the primary dentition (10%) as compared to the permanent dentition (90%). Since it was first reported in 1977, there have been only 37 documented cases in normal children. Most cases involve the primary maxillary central incisors, and very few cases involve primary maxillary lateral incisors. Here, we present a rare case report of a 10-year-old child who had talon cusp on deciduous maxillary right lateral incisor associated with supernumerary permanent successor. The tooth with talon cusp was extracted and the underlying supernumerary permanent successor erupted in the oral cavity. The patient was kept on recall to conservatively manage the supernumerary tooth.

Clinical decisions in dentistry were previously based upon the experience of clinicians, like, “It works in my hands already, and has done so for many years, then just do the same.” Therapies in this fashion were often unpredictable, since the clinician unaware of factors important for success and those contributing to failure. Our desire to keep up to date with new techniques, tests, materials, etc is often tinged with doubt about the claims of superiority of these new treatments or products. Evidence-based periodontology aims to facilitate an approach accelerating the introduction of the best research into patient care.

ABSTRACT

Childhood malnutrition is generally thought of as being restricted to developing countries, but, although most malnutrition occurs there, it is also present in developed nations. During early developmental years of the child, it appears that malnutrition has severe effects on the growth and differentiation of different tissues. Even oral tissues, including the teeth, are very sensitive to the changes in the nutritional supply at these particular developing stages, thus increasing the ill effects on the oral structures. Tooth defects provide a more cariogenic environment and less protection which increases the susceptibility of teeth to demineralization. The shielding mechanism of saliva is also distressed during malnutrition, thus drastically affecting the oral environment.
