Abstract Introduction: Diabetic retinopathy plays a main role in adult blindness. Early identification of diabetes mellitus and its risk factors will result in decreasing complications .The aim of this study is to identify the association between the lipid profile with the diabetic retinopathy severity. Methods: This study comprises cases of 200 number type II diabetes mellitus (100 cases of diabetic retinopathy and 100 cases of normal fundus) and control with 100 number with age/sex matched were examined ophthalmologically. Fundus were dilated and examined with ophthalmoscopy and slit lamp biomicroscopy. Grading of severity of retinopathy calculated by ETDRS classification. Lipid profile analyzed using biochemical method. Results: There was significant association between serum cholesterol level and diabetic retinopathy. Visual acuity was not affected. Conclusions: Hypercholesterolemia may acts as one of the risk factor in diabetic retinopathy and also CSME.
